What is a 24-Hour Urine Magnesium test?
This test measures the amount of magnesium in all the urine you produce over a 24-hour period.
Magnesium is an essential mineral and electrolyte that your body needs for many important functions. It helps maintain normal muscle and nerve function, heart rhythm, blood glucose regulation, bone health, and energy production.
Your kidneys play an important role in maintaining the right amount of magnesium in your body. They filter magnesium from the blood and adjust how much is returned to the bloodstream and how much is removed in urine.
A 24-hour urine magnesium test can provide important information about how well your kidneys are regulating magnesium and can help investigate the cause of abnormal magnesium levels in the blood.
Other names: 24-hour urine magnesium, urinary magnesium, urine Mg, magnesium urine test
What is it used for?
A 24-hour urine magnesium test may be used to help:
Investigate abnormal blood magnesium levels
The test can help determine why your blood magnesium level is too low or too high.
Find the cause of low magnesium
If you have hypomagnesemia (low blood magnesium), measuring urine magnesium can help determine whether your body is losing too much magnesium through the kidneys.
Evaluate kidney magnesium handling
The test can show whether the kidneys are appropriately conserving magnesium or excreting excessive amounts.
Monitor certain medical conditions or treatments
Urine magnesium testing may be useful in people with conditions or treatments that can affect magnesium balance, including some kidney disorders and medicines such as diuretics.
Why do I need a 24-Hour Urine Magnesium test?
You may need this test if:
You have low blood magnesium and the cause is unclear.
Your healthcare provider suspects that your kidneys are losing too much magnesium.
You have recurrent electrolyte abnormalities.
You have muscle weakness, tremors, cramps, or abnormal heart rhythms associated with electrolyte disturbances.
You have a kidney disorder that may affect magnesium handling.
You are taking medicines that can increase magnesium loss through the kidneys.
Your provider wants to monitor your body's magnesium balance.
What happens during a 24-Hour Urine Magnesium test?
This test requires you to collect all of your urine for 24 hours in a special container provided by your healthcare provider or laboratory.
The collection generally includes the following steps:
At the beginning of the collection period, urinate in the toilet as usual. Do not collect this first urine. Record the time.
For the next 24 hours, collect all urine in the provided container.
Store the container as instructed, usually refrigerated or in a cool place during the collection period.
Exactly 24 hours after starting the collection, urinate one final time and include this urine in the container.
Return the completed collection to your healthcare provider or laboratory as instructed.
It is important to collect all urine produced during the 24-hour period. Missing a urine sample can make the result inaccurate.
Will I need to do anything to prepare for the test?
Usually, no fasting is required for a 24-hour urine magnesium test.
Your healthcare provider may give you specific instructions regarding your diet, fluid intake, or medications.
Some medicines and supplements can affect magnesium levels, including certain diuretics, antacids, and magnesium-containing supplements.
Do not stop taking prescribed medicines unless your healthcare provider tells you to do so.
Are there any risks to the test?
There are no known medical risks from collecting a 24-hour urine sample.
The main concern is accidentally missing part of the urine collection, which can affect the accuracy of the result.
What do the results mean?
The result represents the total amount of magnesium excreted in the urine during 24 hours and is commonly reported as mg/24 hours or mmol/24 hours.
The meaning of your result depends on your blood magnesium level, kidney function, dietary magnesium intake, medications, and overall health.
A high level of 24-hour Urine Magnesium
A high urine magnesium level may indicate that the kidneys are losing an excessive amount of magnesium.
Possible causes include:
Certain diuretic medicines
Kidney disorders that cause magnesium wasting
Some inherited disorders affecting renal magnesium handling
Excessive magnesium intake in some circumstances
Certain metabolic or endocrine disorders
Recovery from severe magnesium deficiency
When blood magnesium is low, an inappropriately high urine magnesium excretion may suggest renal magnesium wasting.
A low level of 24-hour Urine Magnesium
A low urine magnesium level may occur when the body is trying to retain magnesium because the blood magnesium level is low.
Possible causes include:
Low dietary magnesium intake
Gastrointestinal loss of magnesium
Reduced magnesium absorption
Dehydration or reduced urine production
Certain conditions in which the kidneys appropriately conserve magnesium
Important note
A 24-hour urine magnesium result should not be interpreted by itself. It is usually evaluated together with serum magnesium, calcium, potassium, creatinine, kidney function, medications, dietary intake, and other clinical findings.
When investigating low blood magnesium, the amount of magnesium excreted in urine can help distinguish between magnesium loss through the kidneys and loss or inadequate absorption from outside the kidneys.
